Care Partnership Agreement

Choosing a midwife is about more than booking an appointment, it's about building a relationship based on trust, respect and partnership.
This Care Partnership Agreement outlines what you can expect from us, what we ask of you, and how we work together to provide safe, respectful and evidence-based care.
By booking an appointment or using our services, you acknowledge that you have read and agree to this Care Partnership Agreement.
Our Philosophy
At Book a Midwife, we believe every person deserves respectful, evidence-based and individualised maternity care.
We are committed to supporting informed decision-making, protecting your autonomy and working alongside you throughout pregnancy and the early postpartum period.
Our role is to provide information, guidance, education, advocacy and clinical care within the scope of endorsed midwifery practice.
Scope of Our Services
Book a Midwife provides Australia-wide virtual midwifery services, including but not limited to:

Pregnancy confirmation appointments

Antenatal care
-
Postpartum care
-
Birth planning
-
Pregnancy education
-
Infant feeding support
-
Prescriptions within the scope of endorsed midwifery practice

Pathology and imaging requests where clinically appropriate

Referrals to hospitals, specialists, GPs and allied health providers

Care coordination
-
Second opinions
-
Birth debriefs
-
Health education and support
The availability of specific services may vary depending on clinical circumstances, location and applicable legislation.

Your Responsibilities
To help us provide safe and effective care, you agree to:

Provide complete and accurate information about your health and pregnancy.

Inform us of any changes to your medical condition or circumstances.

Notify us of any hospital admissions, new diagnoses or treatment from other healthcare providers.

Attend appointments on time.
-
Notify us as soon as possible if you need to cancel or reschedule.

Follow up recommended blood tests, scans, referrals or other investigations.

Ask questions if you are unsure about any aspect of your care.

Treat our team with courtesy and respect.
Telehealth
Book a Midwife primarily provides care via telehealth.
By participating in telehealth appointments, you acknowledge that:

Some conditions cannot be safely assessed virtually.

Technical issues may occasionally interrupt appointments.

An in-person examination may sometimes be necessary.

We may recommend that you attend your GP, maternity hospital, Emergency Department or another healthcare provider if clinically appropriate.
Please ensure you are in a private location where you feel comfortable discussing your healthcare.
Clinical Decision-Making
Our recommendations are based on:

Professional standards

Clinical guidelines

Your individual circumstances
-
Your preferences and values
You remain responsible for making your own healthcare decisions.
If you choose not to follow clinical recommendations, we will continue to provide respectful care wherever possible while ensuring you understand any known risks associated with your decision.
Medicare & Eligibility
Where services are eligible to be bulk billed through Medicare, eligibility requirements apply.
You are responsible for providing accurate Medicare details and notifying us of any changes.
Some services may not be Medicare funded and may attract private fees. Where applicable, these fees will be communicated before your appointment.
Prescriptions
Our endorsed midwives may prescribe medications within the scope of their registration and applicable legislation.
Prescriptions will only be provided where clinically appropriate.
We reserve the right to decline requests for prescriptions where:

A medication is outside our prescribing scope.

Further assessment is required.

It is not clinically appropriate or safe.

Applicable legal or professional requirements are not met.
Referrals, Pathology & Imaging
Where clinically appropriate, we may provide referrals for:

Public and private maternity hospitals
-
General Practitioners
-
Obstetricians
-
Specialists
-
Allied health providers
-
Pathology
-
Medical imaging
The acceptance of referrals is determined by the receiving provider.
Book a Midwife cannot guarantee acceptance into any particular hospital, program or specialist service.
Communication
We aim to respond to enquiries as promptly as possible during business hours.
Please note:

Emails and messages are not continuously monitored.
-
Response times may vary depending on clinical demand.
-
Messaging services must not be used for urgent medical concerns.
Emergencies
Book a Midwife is not an emergency service.
If you experience heavy bleeding, severe abdominal pain, reduced or absent fetal movements, seizures, chest pain, difficulty breathing, signs of labour requiring immediate assessment, or any other urgent concern, seek immediate medical attention.
You should:

Call Triple Zero (000) if you believe you or your baby require emergency care.

Attend your nearest Emergency Department.

Contact your maternity hospital or Birth Suite where appropriate.
Do not delay seeking urgent medical care while waiting for a response from Book a Midwife.
Appointments
Please join your appointment at the scheduled time.
If you are running late, we will do our best to accommodate you; however, late arrival may reduce your consultation time or require your appointment to be rescheduled.
Repeated non-attendance may affect future bookings.
Cancellations & Missed Appointments
If you need to cancel or reschedule your appointment, we ask that you provide as much notice as possible.
Repeated late cancellations or missed appointments may result in limitations on future bookings or the application of cancellation fees where permitted and communicated in advance.
Respectful Behaviour
Book a Midwife is committed to providing a safe and respectful environment for our clients and team.
We do not tolerate abusive, threatening, discriminatory, intimidating or aggressive behaviour.
Where such behaviour occurs, we may:

End a consultation.

Decline future appointments.
-
Restrict access to our services.
-
Take any other action considered necessary to protect the wellbeing of our team and community.
Privacy & Confidentiality
Your personal and health information is managed in accordance with our Privacy Policy and applicable Australian privacy legislation.
We take reasonable steps to maintain the privacy, confidentiality and security of your information.
Changes to This Agreement
We may update this Care Partnership Agreement from time to time to reflect changes in our services, professional standards or legal obligations.
The current version will always be available on our website.
